how to add a customized diagram in toolbar..

Hi...
I have a question about how to add a customized diagram in toolbar...

I use a CSM 19.0..
And I followed steps with referencing the demo in "http://www.nomagic.com/files/viewlets/md_viewlets_customizediagramwizard_viewlet_swf.html?ml=1".

I think I finished all steps successfully(I can find a 'Organization Structure Diagram'), but it didn't add in toolbar, and I can't find a customized diagram in "Create Diagrams"...

Could you let me know how to do...?
